SSI Diver Medical Questionnaire
Every diver on a Freedive Aotearoa course completes the official SSI Diver Medical questionnaire before their first in-water session. It’s short, it’s standard worldwide, and it flags only the things we genuinely need to know about.
How to complete it
- You’ll receive an SSI digital learning invitation by email after your deposit is paid.
- Log in at my.divessi.com. The medical form is part of your pre-course paperwork.
- Answer every question honestly. If you’re unsure about a condition, tick “Yes” and we’ll work through it with you — it’s far easier to resolve before you arrive.
If you ticked “Yes” to anything
Don’t panic. A “Yes” doesn’t mean you can’t dive — it just means we need a signed clearance from a doctor (any GP) before your course. The form for them to sign is part of the SSI digital learning pack. Bring the signed copy with you on Day 1.
What needs medical clearance
The full list lives in the SSI form itself, but the common ones are: asthma (current or controlled), heart conditions, recent ear/sinus surgery, pregnancy, current strong medication that affects consciousness, and any recent injury that limits movement in the water.

For the under-18s
Minimum age is 12 with a parent or caregiver. The medical form must be signed by the parent or caregiver as well as the young person.
